You need to bring a birth certificate, social security card, proof of unemployment stubs, and a government issued photo ID. Requirements change from state-to-state. Go to the Office of Health & Human Services.

It's probably different in every city.

Call directory assistance: 411 and ask for the Food Stamp Office in your city.

If that doesn't help, ask for the telephone number of the Welfare Office in your city.

If your city has a 311 number, call it, they can give you the information you need.

You will probably need your Social Security Card and your driver's license, if you have one. Good Luck.
