One thing to realize is that stray cats have poor nutrition and haven't had proper vet care, so they go into pregnancy with a big disadvantage. Kitten mortality is high even in healthy pet cats and even more so in strays. Possibilities include respiratory infections, parasites or other diseases or birth defects. It's unlikely that it's anything you did wrong. We raise kittens from strays in our shelter - they get vet care and often have their moms or surrogate moms and yet we still lose a few.

There are lots of reasons why a kitten might die in the first few days after birth. Even strictly indoor cats with excellent vet care sometimes loose newborns. I've been there MANY times. I'm so sorry for your loss. It is possible that there is an infection or infestation that will challenge the other kittens. To better their chances of survival take them -- with mom -- to the vet in an enclosed carrier. The sooner the better.

if she has fleas - she probably also has worms...(fleas can carry worm eggs in them).
the kitten (you didnt say how old it was) probably didnt get enough nutrition from mom - unless you have been feeding her kitten food - she wouldnt have been producing good quality milk for them - and usually the weekest die off first.
do not give her milk - its bad for cats - she should have kitten food and should have had it before durring her pregnancy too...
Call your local SPCA or shelter and report finding her - its possible she has an owner looking for her - if you intend on keeping her - great but make you call first and that you eventually spay her...
with over 6 million kittens euthanized every year - you dont want to be a part of the problem...
if you cannot keep her - take her to the shelter (SPCA or ??) because allowing her and her kittens to remain strays is cruel and they will simply be adding to the population of unwanted pets by breeding again.

It could be that the fleas got to the little one. Or it could be there was something wrong with him on the inside: Mother cats are pretty good at detecting such things, and will take the sick and dying kittens away from the others.

Well I'm sorry first of all. If you haven't taken the 3 other kittens to the vet you need to and if you plan on keeping the mother cat get her fixed once she doesn't feed the kittens any more. You need to do this so you can make sure they are all healthy.

the weakest kittens die out first thats why the only cats you see on the street are the strongest because they are immune and healthier. the kitten could of came out not breathing (on the slimy stuff in the mothers birth thingy) or maybe it was just the runt of the litter.

Sometimes, if the mom doesn't take care of herself or isn't getting the right nutrients it affects her babies. I hope the other kittens are ok.
