Hi. I had the same problem of you. Until 2 years ago. I did all those things you said. My hair, pillow, my hands, the food.. everything seemed to be the reason of my acne. I felt guilty of every single thing I did or did not. Until I found a solution. Someone recommened to me a Dermatologist. I was before on one but she did nothing for me and gave me lot of expensive medicine instead. But this dermatologist was different. He "cleaned" my face. Yes, made masks in my every two weeks apointment. And he also removed the 'materia' of every grain (doesnt sounds nice I know). After every apointment my face looked red but the texture was so much better. After 4 apointments I started to have apointments once per month, then every 2 months, now I only go once per year. I dont have any sign I had acne before. And really, my doctor said that none of the things you said are the cause. That is the cause for a temporal acne. But I had acne until I was 24. I wasn't a teenager and if I didn't action that way probably I would still have acne.
So my recomandation is, if you are so desperate as I was, go to a dermatologist, and be sure that you see results in the next 2 or 3 weeks (wont dissapear but your skin will feel and see so much better). Don't stay too long with those dermatologist that give you lot of medicine and don't action on what you have.
This is what I did:
- Went to dermatologist.
- He made me an exfoliant mask before to "work" on my skin, and a mask of Vitamin E when he finished to remove the ugly things.
- I started (and still do) wash my face in the night and morning with Neko, not the soap but the exfoliant lotion.
- The doctor asked me to use every night a gel calls "Benzac AC" (you need prescription to buy it). Is this one: http://www.onlinepharmacynz.com/product/490/Benzac_AC_Acne_Gel.html
Nothing else. My last apointment was 9 months ago. The doctor told me all those things I used to beleive wasnt the cause. He told me I could eat anything I want. Of course I take care of my health, but I dont deny once per month I eat a pizza with friends.

1. - Dont wash your face so often. It dries it out and casues more oil.
2. - Wash ONCE a day with Clean & Clear Blackhead scrub.
3. - After washing, pour some with hazel on a cotton ball and rub on face.
4. - Wipe face with a new clean cotton ball.

close your eyes then you wont see your acne...
but theres a soap i've been using, only use it before you go to sleep its that neutra something face soap.. it's been working pretty well, all you need to do is keep your hair out of your face and stop touching the acne, then it'll help it.
also have a good diet and stay away from oily foods
so thats all i can do to help! hope i did..

Go to a dermatologist. I had horrible acne, I had to go on Acutane (which I think is illegal now... they say it causes depression or something... but I was fine!!!). Your dermatologist can give you prescription topical treatment to use. I still go to my dermatologist once a year to get more treatment, and my skin is almost all the way clear!!!

washing your face really helps and so does doing everything else. i heard that drinking alot of water keeps skin really clear and so does eating eggs.
