It depends. If it's caused by a virus, the damage can go undetected for years, even decades as in the case of chronic hepatitis B & C (HBV & HCV). The liver is a very tough, non-complaining organ. It will take a lot of damage and/or abuse before it will start to show signs or symptoms.
So to answer your question, the first sign of any damage is usually elevated ALT and/or AST enzymes. These enzymes are actually liver cells that are dying off from the damage. But while the enzymes may be elevated, the person may not notice any physical symptoms. As damage progresses, other symptoms become noticeable: fatigue, joint pain, flu - like symptoms.
As I said before the liver is very tough, but it is also very resilient. If the cause of the damage is diagnosed, and the patient takes great care (lifestyle changes, treatment if available, etc), then in most cases people can live a full life. Hopefully any problems can be dealt with before damage escalates to end stage liver disease. ESLD is when the liver begins to fail. At that point symptoms become more serious and a liver transplant may be necessary. I hope this helps you. Best wishes.
