They just want to know if your dog has a microchip or tattoo.

If your dog has a microchip or tattoo you put the numbers/ number letter combination there. If your dog has neither, you just leave it blank or write N/A in that spot.

**If your dog isn't microchipped, I'd highly recommend that you invest in it. It's an INVALUABLE tool if your dog gets lost or stolen.

Well, if the dog has either a microchip or tattoo---enter the number in the spot. If it has neither, leave it BLANK!!!!!

That is what "applicable" means...if it applies.

So you put in your tattoo number or microchip number.. The dog has to be marked for identification by either microchip or tattoo..
