I would!

There's a caveat to that, though. Please don't yell - "Hey! you better get a leash for those yard dogs!" or "Barn door's open!!" across a crowded restaurant, though. In fact, don't make a big deal out of it. Instead, take the person aside, and quietly say "Umm...just wanted you to know, you're about to have a wardrobe malfunction."

If the fly is down, it's ok to let the person know, using discretion. I dont know about the falling out of shirt though. To begin with, I'm not sure it's something I would WANT to prevent. Additionally, it sends the wrong message no matter how you say it..."I noticed you're about to pop out of that blouse, because, well...I was staring at your boobs."

It depends for the falling out of the shirt...

If you think that I intend to look like I will fall out of my shirt, there is no point to inform me, because I know already, and I am trying to attract attention.

Otherwise, If my button is accidentally open, or if I am having any other type of wardrobe mafunction PLEASE TELL ME!!

I would want someone to come up to me and tell me. I would be so grateful. If celeberties did this, Britney wouldn't be in the press.
