Yes. As an employee you have a right to look at your personnel record. If your HR manager refuses, you might want to mention that you do know your rights under the labor law. No matter what state your in, it is a right. It is part of the Freedom of Information legislation.

Absolutely. There shouldn't be a single thing in your file that you should be denied to see. I work 2 jobs. A lot of things in my file are required at both facilities (e.g. TB test results, CPR training, etc.). I obtained a copy of my current TB results from my primary employer and provided it to my secondary one. Likewise, I was CPR certified with my secondary employer and provided that info to my primary one.
If you're being denied anything in your personnel file, I would ask to have the reason put i writing and then seek legal advice.

Yes, it is your personnel data, same as you have the right to see your doctor/hospital records.

Yes in the UK you have the right to see any information kept that is about you. Your company cannot refuse you.
