it's never actually happened to me, but i was just wondering. can you take the shot over, is it a pentalty stroke? what happens? thanks!

If you are on the tee you can replace the ball and start over with no stroke or penalty no matter how far the ball goes. The ball was not in play.

If the ball is not on the tee, therefore is in play, it is a penalty stroke and the ball should be REPLACED, not hit from wherever it went.

Yes, it will count as a stroke, just like if you were to hit a normal shot.

Actually it depends. If you are on the tee and have not adressed the ball yet (meaning you are in a stance to hit the ball) then you may re-tee your ball. ANY other time you must count it as a stroke

Any time a ball is accidentally hit either on the green or in the fairway, you would add 1 penalty stroke to your score replace that ball and hit from the EXACT same spot. The one exception to this is on the tee box. You can always replace it with no penalty on thee tee box.

Heres the USGA definition of a stroke that i found in their rule book

A “stroke’’ is the forward movement of the club made with the intention of striking at and moving the ball, but if a player checks his downswing voluntarily before the clubhead reaches the ball he has not made a stroke.

so if you have no intention of hitting the ball it is not a stroke because you must have the intention of stricking

Yes its a stroke and you would need to place it back where it originally was!! If you dont put it back you will be disqualified when you complete the hole!!!
