Surely you love your son.

This young man has to try to help himself. If he is still drinking heavily and doing drugs, which is his business, what can you do for him? You are blaming yourself for for a grown man's behavior.

Your son's fate is in his hands, not yours. As the mother of a grown man, your only obligation is continue to love him.

Don't let his behavior destroy you.

I am 29 and my mother was so worried about me. I never listened to her but now. My mother shows me that she loves me very much. When I have some problems, she's still be with me. My mother is 49 years old and now I know that I love her very much. I always tell her how much I love her. Maybe you just listen to your heart and your instinct, you will know what to do. If you feel like you need to talk with someone or the doctor about your son...go for it.
