Well this questions can have 2 possible answers.
Spicy doesn't not alwyas mean HOT.
Cajun seasoning and or blackening seasoning is a highly seasoned as in spicy as in a lot of flavor.
If you asking about spicy HOT I would have to say curry as a dish, and scotch bonnet pepper (which I did as a bet, and regretted it). The spiciest as in flavor would be blackened redfish.

I was in a highly recommended Thai restaurant in Chicago once and since I wasn't familiar with the cuisine I asked the Thai waiter to order me what he thought would accurately represent the cuisine.
Most of the food was painfully spicy and I haven't been to a Thai restaurant since.
