They don't. They have to wait just like everyone else.

No, its a judgment call on the emergency vehicle driver if he thinks its safe to pass. Every state has laws on the books that make emergency vehicles exempt from traffic laws while in the performance of their duties.

Those same exemption laws also say that if the act is considered as reckless and / or causes injury or damage that the operator can be held both criminally and civilly liable.

I served as a paramedic and firefighter for almost ten years. When emergencies happen often dispatch (the 911 center) can notify the train company to do multiple things to help expedite transport through such situations. Many of the train companies here will stop the train before it hits the crossing or in the crossing and unhook part to let the ambulance through. Ambulances by law are not allowed to run through the flashing track guards. Most of the train systems are very helpful when notified in advance so that the train itself can simply be delayed. This is not to say that sometimes people don't get notified quickly enough. There have been very unfortunate delays in care waiting for the train to pass. I however was very luck and grateful that most waits were very short.

Most of the time, in our small town, our railways are loading yards, meaning the trains are only moving at 5-10 mph. So when the rail workers see us coming, they cut the train apart, and let us through

They cant get through....most places have alternate routes built in to get around train tracks in a situation like this. Otherwise, they emergency vehicles have to wait.

They sit there like every one else. Have you ever seen what happens to an ambulance that has been hit by a train?

they have to wait.
If there is an alternate way to go they will usually try to avoid railroad crossings.
