It could be a number of things, bladder stones the one which sticks out the most. You really need to take him to an exotic vet, use the following site for a vet locater:

You are not alone! My little man was having bloody urine, (can appear as though it is related to the 'butt'). I took him to the vet, and a course of antibiotics, (Bactrim) and he is fine. The vet did an ultra sound on his belly, and found no abnormalities-they need to do this to look for stones. The vet also took a urine sample from him and cultured it. This is quite expensive, upwards to $200.! Some vets will work out a payment plan with you, ask if needed. The antibiotics only cost $6.. I also gave him an extra sippy tube of dilute organic cranberry juice. This can be found at a herbal store, it can't have sugar added to it. I really watch what he eats, and his bedding now. When you have a male, all sorts of stuff can get caught in his foreskin. Check his penis when you hold him. If he is a long hair, trim the hair around his penis. This was my experience, and yours can be completely different, but I hope this helps you. A sick piggy doesn't automatically mean that you have to 'put him down', but it will require effort and commitment on your behalf. The antibiotics must be given via syringe twice daily. Your pig will not like it at first, but he may start to look forward to it as a treat. My pig got his meds. every morning and night for 14 days. It was bubblegum flavored, so it was kind of a treat. He still resisted the syringe in his mouth! Good luck, you and your little man will be in our thoughts!

Maybe it is that season when guinea pigs want to... mate. Does the small and large guinea pig fight each other? If so try bringing the small guinea pig and the medium guinea pig inside together sit them both on your lap. Give them a carrot to share. If one bites the other take the carrot away. Keep trying this until they get the idea that they don't get the carrot if they fight. Also try that with the Large guinea pig and the medium.

I definately agree that you need to get to a vet ASAP. I agree that bladder stones are a possibility. Usually surgery is needed for these. However, HOPEFULLY it is only a Urinary Tract Infection which is treatable by antibiotics.
**Since your pig is a male, he will not have the "reproductive" problems that the first answerer suggested. Ovarian cysts do occur in female pigs. Most of the time they are removable by surgery.

Yes, take your piggie to an emergency vet. The only help you can get online is people's personal experiences since most of us aren't vets and can't see your pig anyways.
Could be a scratch, tumor, could be anything....harmless or life-threatening, but get it checked out.

This happened to my very first guinea pig. Although she was a female. Her name was Terrela and she was only about 11 months old.
Well, she was bleeding on her privates & she ended having a tumor in her female organs. So, I had to put her to sleep.
You need to take him to the vet. Or he will end up dying painfully. You don't want him to suffer do you?

Most common cause is bladder stones. They can cause obstruction of urine. They are painful, and are very serious. You really need to get him to a veterinarian who is experienced with guinea pig medicine...he may need xrays and medications to help treat. Sometimes surgery is necessary.

okay this will sound stupid...but are you shure that it is a boy...when i first got mine they said it was a boy...but then i found blood on my piggie and in its cage...to later find out that my piggie was not a boy but a girl and she was having a misscarrage...it was awful...i felt horrible and blamed it on myself...there is a verry small chance that this is what is wrong with your piggie bu anything is possible...hope your little friend gets better...good luck!!!
