The the largest single category of death and injury in US service members in Iraq is 'DNBI' (Disease and Non Battle Injury).
The fact of the matter is that if a soldier has a heart attack in Iraq - it is counted by the news media as if it were a combat death,.

The IED is a major factor. What isn't discussed much is the high rate of deadly disease and infection. Some of these middle eastern fungal infections cause death and disability...figures that don't show up on the casualty score card.

Specifically Vehicle-Borne Improvised Explosive Devices (VBIED) and Radio Controlled Improvised Explosives Devices (RCIED).
VBIEDs are basically car bombs. RCIED are used in roadside bombings. They plant them in or on the roadways and wait for a Hummer to drive over it and then detonate it.

But remember, most of the combattants we are fighting do not have access to weapons factories and an industrial infrastructure to make their armaments.
So, by definition, almost all of the weapons they are using are "improvised".
