What makes you think he knows? Have you told him? Do you flirt openly? He probably doesn't know unless you have made it really obvious to him. Clearly, if he doesn't know, there is nothing unethical about him treating you. Even if he does know, or suspect that you have a crush on him, as long as he does not become involved in a relationship with you, there is nothing unethical about it. However, many docs fear lawsuits from patients who they believe to be romantically inclined toward them, and they can choose not to treat those patients. (They have to provide you notice that they can no longer be your physician, and advise you to find a new one).
The only way your physician can have a personal relationship with you is if the professional one you have is completely severed. (many psychiatrists swear to NEVER become involved with a past patient). Bottom line is that docs are pretty normal people, and you may make him uncomfortable (just like any other man) by expressing an interest in him.

You said you have a crush on him, which sounds like it's one-way only. It would be best for him to give you a 10-day notice instructing you to find a new doctor because your crush on him makes treating you very difficult. It's not unethical for him to treat you, but it is inadvisable.
A doctor can terminate the doctor-patient relationship for any reason he deems valid. He is required to give you notice in writing and at least 10 days notice so you can find a new doctor.

family doctor's experience:
i really dont care if a patient likes me or not, the only thing that i need is constant appointments, so i can keep practicing. If you have feelings, it is good for him because you will be a long term patient.

this situation is known as transferrance. every doc out there has tons of patients crushing on them. as long as they ignore it and don't act on it (which is usually the case) there isn't a problem. if you get to be a pain in the rear, however, you'll be outta there.
