It depends on what follows. If it's something that required thought, I'll give the question thought before I answer it.
If it's something designed to get people on the defensive, I take a few deep breaths, THEN answer. Or sometimes I decide that it's not worth my time.

I give it some thought if I answer them!!! I am not that defensive a person...I have gone through the valley of the shadow of self awareness and don't have much to defend anymore...

Depends on who asks. Some people will judge you if you say anything other than "fine, and how are you?". Most people don't really want to know, don't really want to hear your problems if you have any, they just use that phrase like "hello".
