Insert your floppy disk, head to My computer, do right-click over your floppy drive, select Format, then check 'create an ms-dos disk' & format.

Depends on what you mean by it.
The plainest form, just put a disc in A: (assuming you have an A: drive as your floppy drive), right-click A: from my computer, select FORMAT, and click "OK", and wait. That's from Windows (almost any version).
Or from a DOS prompt, FORMAT A:
If you mean a BOOTABLE floppy, that's something a LITTLE different, and I'd have to ask what for, and why. It's normally FORMAT A: /S, but again, there are a few variations and a lot of additional concerns such as AUTOEXEC.BAT, CONFIG.SYS, drivers, and such.

go to bootdisk.com and download any dos disk you need. You should have a copy of windows 98SE boot disk it comes in handy sometimes. When it boots up you have access to your roms and dos. IF you want to make a bootable floppy dos disk you will need a copy of windows boot disk boot upon the floppy disk then type format a: /s then return it will ask for a floppy to be placed in the floppy drive then return.
Do not place the boot disk back in the floppy drive use a new or formatted disk.

If you have the appropriate disc formatting software, it's actually quite easy. I own a Mac using Mac OS X and Disc Utility, and I can write discs in Mac OS X Journaled or Windows/DOS format. Try doing a search in a handy dandy search engine for "Disc formatter for Windows (or the OS you're using)". You should get many hits, but always (repeat: ALWAYS) scan it for viruses if it's from a company you know nothing about because it might just be a bundle of viruses you're installing on your computer.
