I am ready but want to make sure I do it the right way

Just say, in your own words, something like this:
Lord Jesus, I am a sinner and in need of forgiveness. Please come into my heart and lead me in your way. Thank you, Jesus. Amen.

Pray something like this with your heart open and ready to receive.
Father I come before you a sinner and I am sorry for all the things I have done to offend you.
I accept your word that through the sacrifice that Jesus made on the cross I can be forgiven. I ask for that forgiveness and for Jesus to come into my heart and reveal himself through the Holy Spirit.
I ask that you guide me in learning your ways and to strengthen my faith.
Grant me patience Lord to hear the small quiet voice of Jesus.
In His name I pray. Thank You for my salvation and the blood of the cross that washes my sin away. Amen

All you do is pray, and this is what you say Dear Jesus, I am sorry for all the sins I have done. Please come and live in my heart! Wash away my sins and live within my heart! I want to be in your presents! I want salvation and live with you forever and ever! Amen! That is what I said when I asked Jesus into my Heart! But, go to your Mom and Dad. They will take you to the preacher to get Baptized! Hope you Get saved!!!

The right way is to speak from your heart, talk and let the words come out without rehearsing them. Plain and simple.

With a humble and sincere heart desiring repentance from your sins, you simply tell God the Father that you are sorry for all your sins and accept his son Jesus' crucifixion and death as your sacarifice for those sins.

All you have to do is pray, and invite him in, and the rest follows, once you think you have made contact.
Jesus himself said this was best done in solitude, and not with the intervention of any church.
