You mean aside from the fact that they are different parts of speech? I would say that altered perception was a more-or-less permanent condition, while mild altering drugs only last a fairly short while. I did quite a few mind altering drugs in my hippie days, and what happened was not only incredibly altered perception for the period of time when the drugs were in my body, but a permanent change in the way I see the world that has persisted, even though it has been many, many years since my last acid trip.
Also, altered perception can be achieved by many experiences, whereas the term "mind altering" is almost always applied to drugs, rather than other techniques.

Both refer to biochemical reactions in the brain; however the time 'mind altering' is most often used to refer to an artificially induced change whereas 'altered perception' is more usually reserved for states achieved naturally (eg; near death experience, meditation).
