Don't you have a passport from your country of origin???

That's all you'd need. A valid passport and the Green Card.

If you have your green card, you are ok to return to the U.S. Your valid foreign passport will be needed too.

Canada accepts visa waivers from about 50 countries, but since I don't know what country you have citizenship from, I couldn't say whether you need a visa. Contact Immigration Canada.

You need the passport of the country of which you're a citizen (which you needed to get the green card). They may also ask to see your green card. Certainly when you're headed back, they'll want to see it.
