How hard would it be to change a nissan extended cab truck from a standard to an automatic transmission?

major undertaking, don't do it unless there is and over whelming need to do so. It's time consuming and expensive.

Easy trade it in and buy another truck. Your question lacks essentials like year and miles on the truck. The automatic can be a problem even the throttle cable is different and the throttle chamber has a second cable that go's to the transmission. I just replaced a 2000 truck transmission today with the same automatic. Owner towed it from Georgia with all the wheels turning the trans failed from lack of lubrication.There are lots of differences.
