Think I've heard on MSNBC that it is a requirement of all Americans citizens to when travelling abroad to produce a passport. Think when my family was over in Canada a few years ago, they were asked to show proof of identification, being a passport. So I would figure, just to be on the safe side, you should take one along if you have.

The US and Canada are in some sort of dispute over fingerprinting. Under Canadian law, only criminals are fingerprinted, but under US law all visa applicants, criminals, and government employees need to be fingerprinted.
They are going to be issuing some sort of card good for North American travel soon, but right now a passport is needed (or at least highly recommended to avoid problems).

Only for flying if you are driving into the states not this year but next year you will also for cruses if you crose over American waters than you need to have a passport to go on the cruise.

Yes, a relatively recent change, as many, rooted in 9/11. There is a huge backlog of processing currently, if you need one, get the application in ASAP! You can get information and submit it at your local Post Office.

yes. beginning from Jan 1st 2007, all Canadian and U.S. citizens are required to have valid passport upon entry to US. this is one of those measures the DHS adopted after some criticism saying US is too soft on its Northern boarder.
