I have a friend who weighs about 300 pounds, and she has a problem that maybe a dermatologist could help with. Because of her weight, her abdomen sort of folds over , and underneath it--from hip to hip and across the tops of her thighs--she has this terrible problem with sweat accumulation. She uses antibacterial soap to bathe with, and has used creams (Desitin, cortisone-types, and medicated powders--sometimes alone and sometimes the powder on top of the cream) to keep it dry, but nothing has worked. It is so heavy sometimes she can even scoop it up on her fingers ( it is a pale yellow color and has a fairly strong odor). It looks like a first-degree burn on her skin. Prior to her hysterectomy in 1997, it happened only before her period. Now it's an everyday occurrence, and it gets very sore.Within the last year, she has had it behind her knees as well (it has blistered & even bled there). Any suggestions about the cause or how to cure it?? Medical help appreciated.

I'm not a Dr, but I am a CNA and we are trained in how to care for skin. Putting lotion on an area that is getting sweaty is a bad idea, and putting powder on top of lotion will just make a paste. She needs to get some powder, Gold Bond in the yellow container is great, and powder the problem areas, after bathing and drying the skin completely, every day. She may have to reapply powder on hot days.

It's important to keep the area clean and dry.

After she takes a really good shower, have her lie down somewhere private. She'll need to spread her legs wide so her thighs don't touch and hold her abdominal flap up, off her pelvis.
The longer she can stay in that position, the better chance her skin will have of getting dry -- positioning a table fan will help.

If she can learn to sleep on her back, with her thighs apart, and her abdomen raised, that will also help.

During the day, she should make it a point of drying herself frequently. When she goes to the bathroom, for example, she could take paper towels and blot all the areas under the skin flaps.

She may want to go to the dermatologist and get a stronger steroid cream. But, like one other person said, it may be a yeast infection. She could try some over the counter anti-fungal cream, like athlete's foot cream, to see if that works (may take a few days), and if that doesn't work go to the derm.

This is a really common issue with overweight persons.
The rash could be a yeast infection or bacterial infection, only a doctor visit will be able to tell.
After treatment, the area needs to be kept clean and really really dry or the infection will come back

she should go see a doctor but i know you dont wanna hear this but losing weight will save her the trouble
