Maybe a nice framed photo of the kids? You could write something nice in the card about how you'll remember her family and keep a picture for yourself too...?
I think whatever you get you should get a card and write a personalized message. That will convey the most.

How about a fruit basket, or a gift card so they can purchase something for there new home. Or maybe if you have any pictures of the kids why don't you go print some off and have them framed and say here's something for your new home maybe put little quotes on the pictures just remember anything you make will come from the heart

I think a card would be wonderful and she would probably save it. Especially if you put all your favorite memories of the Kids growing up. If you want to give an actual gift maybe a game or something the kids could play together or with their mother. I am sure the mother feels the same about you and would appreciate the written memories more then any gift though.

If the mother wouldn't mind, you could take a pic with yourself and the children. Frame it and give it to them, not only is it a nice sentiment but the children can always look at it to remember you. Maybe you could give the children a special toy or stuffed animal or a book, something that you only know is a special intrest of theirs do to the time you spent with them. Same with mom or another thing I always like to get as a single mom is a basket of bath items oils, bubblebath, salts, lotion etc.. It may sound silly but as a single mom, "pamper" items feel like someone knows how hard your job is.

Well do you want to give them all something or her. You mentioned her several times so I wasn't sure. I would make something or give something from the heart that will be remembered. If you have pics you could make a collage...or buy new house warming gifts like sheets or kitchen stuff, although that would be less sentimental it just depends on which way you want to go with it.
