Reality is that which in fact exists.Our perception of reality is limited by the distortions introduced by own psyche, our ability to experience it fully with the 5 senses we possess (and may be it is arrogant of us to believe that it can be experienced fully just with the 5 senses we have), and also by our ability to assimilate and understand or correctly interpret that which we experience (which is itself dependant on the body of experience we have built up) Reality does not require description or interpretation to be real.
Truth is more abstract. Truth is the correct description of the nature of something. The quest to understand reality, is also the quest to determine the correct description of the nature of reality, which is why we tend to confuse the two. Both require the need to increase our experience of it, and purify ourselves to make sure we do not distort that which we experience. Reality reflects the varying conditions and circumstances that characterize our world—right or wrong, they’re all a part of real life. Truth, on the other hand, is objective, eternal and absolute. It is grounded in the Word of God.Truth can set us free ,sanctify us and purify us . Reality cannot. Truth gives us a reliable set of unwavering parameters to live by. Reality is affected by time and manipulated by the agendas of man. It knows no boundaries of acceptability.

Truth is subjective. My truth is not your truth. My reality is not your reality. A fact is something you know you can prove, but sometimes cannot. The difference lies within the thoughts of the asker. Is your reality a fact? Or is it merely your perception of the truth?

The truth is always the truth, peoples perception of it doesnt change what it is. what happened is what happened. people lie to themselves a lot, tho, and hope they can get others to believe them. That still doesnt change the truth. sometimes no one ever really knows what it is though. Just like religion, for example A whole lot of people believe a certain thing, and want everyone else to believe it too, but that doesnt make it true (or not true) How will we ever really know?
A long time ago, some people wrote down a bunch of stuff, claiming it was true, But just one person involved with that writing, could have been lieing, and now, the whole thing could be corrupted. And here are millions of well meaning people possibly livivng a lie, But we dont know for sure, do we? Unless we build a time machine to check facts on all kinds of possible lies, we will never know.

Where do things like truth and fact are different come from.... It's like saying intelligence and wisdom are two different things.... Believe it or not it comes from the influence that christianity and the bible have on our cultures and societies...
The battle has been going on for so long (2000 years) that the Christian position of "it doesn't have to be proven to be true" is acceptable to far to many of the population.... Really it's time we look at the negative affect this religion on our planet.... some things "spiritual" are not seen but have an effect and leave a physical imprint or evidence... the problem is the people who knew how to read them have been murdered by the christian religion... catholic crusades and inquisitions... burnings and hanging of heretics and witches... what was worked with nature, what replaced it was catholic and christian rhetoric... and it's effects are enormous and devastating to our natural world... it's time to take the blinders off and take a good look at the negative impact this religion has.

'Truth' is an entirely subjective affair, contrary to conditioned belief, We still think that we are subjective beings in an objective Universe, it is completely the other way round, we are objective beings in a subjective Omniverse.
'Reality' is whatever each and every one of us chooses to make of it. Shakespeare once wrote "All the world's a stage, and the people are but the actors on it" This is an incredibly profound declaration, if one can get the the deeper levels of meaning. To each of us our own individual 'play is unfolding, we are the scrptwriters, the director and the stars of our own 'productions', and in turn everyone else who comes into our lives 'plays' a role that we have defined for them, all the while we are 'playing' a role in their 'play', written and directed by them.
There are no 'facts', all relative data in this realm is transient. What we describe as 'facts' may be true today, but false tomorrow, and we should be much more circumspect about how easily we throw the word 'fact' around.
The 'difference' is an illusion, in order to create the appearance of 'separation' that actions in this realm require.
Truth is different because it all depends on the observer's perspective, seen from one 'side' a lump of rock may just appear to be a lump of rock, seen from the other side, which has been split open, one may see that the 'rock' is, in 'fact' a crystal node, containing the most fabulously beautiful coloured mineral crystals, all shapes and sizes, glittering the rainbow colours of refracted light.

I'm going to answer the first question last.
We all have our own 'realities' which our brains crystallise out of imminent possibility.
If most people agree on a particular reality, or if there is concrete evidence for it, it may be considered to be a fact.
Facts may be 'true'. (They may not, of course. We may be deceived).
Truth, though, with a capital T, transcends all of that. No, I don't think it is a personal perception so much as perception which transcends the personal.

You have simply asked a question which can't be answered in so many words. However I'll make an attempt.
Words have limited potential in knowing or understanding fully what it is.
Even to see yourself in full we require a mirror.Reflection is perceived to be 'I' and it is more a matter of identity.
Can you go above identities of certain things which we hold exclusive to us like even gender?
For example golden ornaments say, a ring or necklace they differ very much in shape or how it is presented.But the basis for them is gold which you get when you melt them.
Is it possible to dissolve me devoid of identities? Saint Ramakrishna Paramahams asks can a salt doll measure the depths of the ocean?
So TRUTH may be too big or complex to handle with our limited potential.Even our sense organs can only do what is expected of. That is an eye can only see, can it hear?To come to your main question :Truth can not be of perception since it is a matter of subjectivity.
