Yes, I have. Now I have become better at catching it. Mostly when something is say 6.40. I give 11.40 and wait for my change. The five bucks always seem to get lost. Now I make the cashier count the drawer and insist I want my change.
And not even to metion idems that fall apart as soon as you take the wrapper off
