Could be "dry socket," which, when it occurs, occurs between 3 to 5 days after. He should go back to the dentist so the dentist can apply a medicated dressing in the sockets that will immediately take away all the pain.
Dry socket is the lack of an effective clot forming. If your boyfriend was up and about and too active after the extraction, thinking he could live his normal, vibrant lifestyle immediately after the procedure, then that would be a good evidence of dry socket.
When they say lie down and just rest ... even if you feel normal, it's important to lie down and rest. When they say don't suck on a straw or spit, and don't eat certain foods, follow the advice religiously, regardless of "what feels fine." Tooth extraction is an subject best dealt with conservatively and cautiously. If they say rest two days, might rest three. Etc.

Dry socket often is something that can happen a few days after a tooth being removed. It can be quite painful. He should contact his dentist and make him aware of it. Also, rinse mouth with warm salt water in the meantime. Here is a page with info on dry socket to verify and get help.

The shots are worse than getting the teeth pulled sometimes. I had really bad teeth all thru school, had quite a few out along the way and finally had them all pulled soon as I turned 18. The shots are worse than having the teeth pulled sometimes, but usually feels pretty much back 2 normal in a week or 2 at the most. Usually ok in 4-5 days.

He may have lost his clot(dry socket), have something stuck in there. Normally the pain only lasts for about 3 days. Warm salt water should help.
