Perhaps a framed collage of her life that celebrates who she is and what she has accomplished. Or a bird feeder for outside her window. Or a gift certificate for a handyman to fix all those little things around the house, or a maid. Go out for a manicure together. Host a party for her far away friends. A magazine subscription like Large Print Readers Digest, Guideposts or Birds & Blooms. Or Books on tape. If she has arthritis in her hands, check her kitchen drawers and see if she needs some new utensils with built up handles.

Srapbooks or a quilt considering grandmothers usually make those for others but might not ever get such a considerate and personal gift.
Also, the older generation enjoys social events such as going out to dinner with a friend, travel, plays etc. lol - we all enjoy those things it's just that we have to squeeze the time in sometimes.

My grandma loves it when I get her things with the word Grandmother on it. One year I got her a nice throw blanket with a poem on it about a Grandmothers, that is her favorite. You could also get something ingraved, a shirt, magnents, etc.

If she is unable to get around too well, a portrait of yourself, your family, her loved ones is always appreciated by our elderly family members/
If she can get out, take her lunch at a nice restaurant and/or have your portraits taken together.
A lot of hotels are now doing "proper afternoon teas", try taking her to tea.
Treat her to a manicure, or have her hair done. This always makes anyone feel good.
Have a wonderful day and Happy Birthday to Grandma!!!

a framed picture of her kids or any part ofher family - the grandkids etc. what about a shirt that says "world's greatest grandmother" other than that you have to go with her interests - does she like gardening or knitting or dancing or ...
is she religious, maybe a bible or religious book. if she likes cooking maybe a pan or utensil.

Maybe you could take her out to eat for a nice dinner! Another thing you could do is pay a bill for her, or give her money! If she is very old things don't matter as much as they use to! That's nice of you to think of your grandmother!
