Take the #1, 2 or 3 uptown one stop to 42nd St-Times Square. Transfer to the S (42nd Street Shuttle) and take that to Grand Central (1 stop).

The current fare is $2.00. If this will be a one way trip (Penn Station to Grand Central) then you will have to purchase a single ride MetroCard from one of the MetroCard vending machines located near the turnstiles (cash, debit or credit). The station booths (formerly known as token booths) do not sell single ride MetroCards. The minimum purchase at the booths is $4 (2 rides) cash only.

Note: If the booth is painted red/maroon then you can only make a purchase at the MetroCard vending machines.

there is a shuttle subway.
best thing to do is ask one of the Amtrak or NJ Transit customer service reps for the directions to the subway line.
I've done it but was with a group that know what they were doing and I just followed.
Takes 10-15 minutes total

I believe you can use the MTA to access each station. Check their website for more information.
