It is a statement you recite that says you are truly sorry for the sins you just confessed. Without it, a confession is nothing more than having a friendly chat with a priest. With it, the priest understands that you are really sorry for the sins you confessed, and absolves you.

An Act of Contrition is a prayer said after a person is finished saying their sins.

O my God, I am heartily sorry
for having offended Thee,
and I detest all my sins,
because I dread the loss of Heaven
and the pains of Hell,
but most of all because they
offend Thee, my God,
Who are all-good and deserving
of all my love.
I firmly resolve, with the help
of Thy grace to confess my sins,
to do penance
and to amend my life. Amen.

An Act of Contrition is a prayer made by the penitent. The first part of it is essentially an apology: "I am heartily sorry for having offended you, Lord, and I detest my sins because of your just punishments, but mostly because they offended you, Lord, who are all good and deserving of all my love."

The second part is a promise, as well as a prayer for strength: "I firmly resolve, with the help of your grace, to avoid sin and to avoid occasions of sin."

the prayer is listed above and it's basically being humble and contrite before God - telling Him you are sorry and need His help to avoid sinning again.
