You hold it so that all the air is coming through the material being smoked, and brings a lot of the smoke into your lungs. You release it when you have almost as much as you can hold, so that the extra air pushes the smokey air all the way into your lungs. If the pipe stops drawing easily because it is getting somewhat clogged, you can sometimes make it work better by opening and closing this carburetor hole.
