Yes, the do. They carry what is known as a Diplomatic passport and have to go through the same procedures to enter other countries the same way everyone else does. Case in point, when Fidel Castro wanted to come the States, he was denied a visa. When he wanted to go to the UN, he was issued a conditional one.

Yes. They still need a passport. It sounds really far-fetched, but if they ended up someplace where people couldn't identify them, or if they were burned to death or something, the passport would be a record of who they were and that they were legally in the foreign country. I'm not sure that they have to get the stamped, though.

In the US, the president needs a passport. Only a national sovereign doesn't need one. For example, the Queen of England doesn't need one, since technically SHE, as the sovereign, issues passports through the government.
