there many be legal issues related to your allegations.
what is the evidence since it is not easy to simply lay this claim?
the profession has a regulatory body that takes care of poor professional performance and conduct, and your friend takes the risk of malpractice if the proper procedures are not followed.
additionally there are checks and balances in the profession that sometimes a few manage to get through the cracks and adverse results happen.
the best thing to do is to have a conversation about your feelings and ascertain the seriousness of his conversations about his practice with you. Professionally, these should not be discussed outside the practice hours unless it is between like professionals at a meeting, conference or research venue. There are obvious confidential standards that could be violated in discussing his diagnosis with anyone outside the profession. Authority from the client(s) must be obtained to discuss the ailments and treatments.
it does not hurt to approach the subject in parts - first about his openess to discuss the treatment of patients, second about his time at the college and how he managed to be certified, and finally to ask the person to clear the matter up so that your concerns (and the other person) is not an issue.
show your concern by starting communicating is one good way to show that you are a real friend and help.

He is risking other people's lives. He could be responsibly for killing someone. That would hurt him worse than ratting him out. You need to tell someone. He doesn't know what he is doing.
