I have been working for a physcian for a few years now. In 2007 I had a child....I want to claim him child on my tax forms...but my employer has never had any of their employees fill new ones out on a year to year basis? is this required? and does a new form have to be filled out in order for me to claim my child for 07' taxes?

You are referring to a W-4 form on which you put down how many exemptions you want to claim, and what your marital status is so that the payroll dept can figure out the federal and state withholding on your paycheck. If there's been no changes in # of dependents and/or marital status then there's no need to fill out a new W-4. Unless you've been either getting a large refund and you want more in your take home pay, or you've been owing a lot at year end to the irs and you want to fix that.

You are talking about a W-4 form. They dont have to be filled out each year UNLESS you claim exempt from witholding. If you want to change the exemptions then you must complete a new W-4. See your state also. PS You can download the form W-4 at irs.gov and give it to your employer.

The employer is only required to ask you to fill out a W-4 once, when you're hired. After that, it's up to you to file a new one if your withholding situation has changed. Until you fill out a new one and give it to your employer your withholdings will not change.
