I'm having the same issue with my zucchini. I've also heard alot of other people with the same issues this year. It may have been some bad seeds or something.

If you do have squash growing to 2 inches, it doesn't sound like a pollination problem, but it does sound like a deficiency in the soil. It could be a lack of calcium or even could be too much nitrogen. Your best bet would be to take a soil sample to your local County Extension Agent or even a local university if they have an agriculture dept and have the sample tested.
Another factor to consider too is the consistency of the amount of water that the plants are getting. Too little or too much while they are forming squash can also be detrimental.

hello there!
OK here is my answer, first look around your plants, and look for deficiency's, for example lack of direct sunlight, lack of water or too much water, no bees? for a good pollination!,or lack of food like nitrogen, phosphorus or potash, and the last one how many plants of the same variety you had planted closely enough to get good pollination?
is the fruit is set and few days later fall, sound to me like potash deficiency.
my advise is get a soil analysis , it will tell you exactly what you need to do to correct the problem with nutrients deficiency's in the soil, make sure yore veggies receive at least 4 to 6 hours of direct sunlight, and make sure to water well in the mornings, never in the evenings...

12 inches is about the average length for a squash. If the get any bigger they will be tough. a smaller sqaush has a better taste. Bigger isnt always better.

It sounds like a pollenation problem. The fruit will form and die. Try hand pollenating some and see if that doesn't solve the issue.
