Form is arbitrary. Larry Bird is one of the best shooters of all time and had an ugly shot. What you need to do is simply, shoot... all the time. Your form will develop itself. Some basic rules... square up, shoulders parrallel with the basket, shoot "up" not "at" the basket (the ball should drop in), make sure you have solid rotation on the ball (proper follow-through), relax-- you should get to where shooting is a muscle memory related instinct. You should never think about "how" you shot it, your instincts should take over. The only way to accomplish this is to practice. Shoot 500-1000 shots every day as quickly as you can.

you want to establish a shot that minimizes the amount of energy you use to lift the ball to the height of the basket, jumping should be enough to get the ball a desired height

focus on using your wrist to direct the ball in the direction you want

dont EVER use your palms to release the ball, and dont EVER use your thumb on release of the ball (try not to use your pinky either, sometimes its unavoidable though), only the middle three fingers should accompany your wrist motion

at first, this shot might feel VERY uncomfortable, keep practicing at the free throw line at the beginning until it becomes natural, then gradually increase your distance from the line

practice makes perfect

(Added: form actually matters alot, if your shot relies too much on your arms for its vertical climb, it exponentially decreases your chance of making shots off a screen, off a step, fadeaway, etc, because now you have extra momentum to compensate for, where as a wrist motion guarantees control and a quick release - larry's nba days were at a much slower tempo than today's basketball)

shoot more, and when you're done, shoot more. Seriously, repition is the only way. Focus on fudamentals as well, so that you don't use too much energy try to go all And1 on everyone, that way you will have more "legs" for your jumpshot. Trust me coaches and scouts much prefer someone with a spot on jumpshot and solid fundamentals...why do you think the professor, sauce, AO and all those cats aren't in the league, while Luke Walton, Luke Ridnour and Adam Morrison get paid millions and have like 1/3 of the athletic ability...

You'll take thousands of 3pointers to get good at it. There is no right or wrong way to practice shooting. See if you can net 10 in a row from the 3point line. If you can then move back a foot and try to net 10 in a row.... If you can net 10 3pointers in a row then you have mastered the shot to the best of your ability.

if you are really commited to improve ur shot, go to gym or outside and just keep working on your shot. If u wanna change ur form then make sure your right arm is at a 90 degree angle and ur left hand is on the side not on top or bottom and bend those knees

I think this is a great question for me. I loved to shoot 3pointer. i was the top one in my high school. all you have to do is pratice them. start shooting your jump shots, and every one you make start moving back, eventually you'll be hitting 3's in no time. once you are at the 3 point line set some goals like you want to make 30 a pratice. don't get frustrated if you don't get it right away, it will come to you. i hope this helps. good luck.
