I have a 1998 escort ZX2. I have replaced the belt and tensioner and the belt still comes off. It may go a couple of days or a couple of weeks before it comes off again. Usually happens while I am at a stop and engine is at idle.

I am willing to suspect that either the pulleys are having trouble gripping the belts or your serpentine belt has too much lubrication on it. This happened to me once and after the mechanic tightened the pulley latch and the belt adjustment, everything was fine.
