No fears at least in India. Our last names are generally the family names which run through genrations unless specifically changed. The first name is given bey the parents who can be counted upon giving some distinctive name different from the family name

What happens? Nothing. Your first name is the same as your last name. That's it. Of course, I'd have to say that to do that, someone did not have very imaginitive or creative parents. It's like calling a spotted dog "Spot". Come on people. You can name your kids better than that.

?? nothing 'happens', except maybe some exasperation at your parents for the choice.
Since I assume you intended for your question to go to the genealogy area, as it did.. once you get past the exasperation, you move onto to identification of your ancestors, and learning about them- their names, details of their lives, etc.
By the time you get to the grandparents, it is no longer an issue about your name. IF you are fortunate enough to find observations, maybe biographies... it is possible you may find a family history of someone with a sense of humor.
