Number one reason why people fear getting stuck in an elevator is suffocation, so why can't they just add some holes somewhere so you could breathe.

I am an HVAC installer and every building I have worked on has some sort of elevator ventilation. The biggest issue with elevators is that they are a breach in the fire protection system of a building and it is very expensive and difficult to comfort condition a large vertical shaft without causing fire safety concerns.

Could a self contained a/c unit be installed on an elevator..yes it's possible but the expense to install and operate are not high on the priority list for building owners especially since each time the door opens, conditioned air from the lobby enters the cab.

There are vents ate the base of the cab and a fan at the top to keep fresh air entering at all times and at least one vent at the top of the shaft to expel excess heat.

There is ventilation in elevators. All of them. You may not see it, but it's there, for exactly the reason that you mention.

There is always ventilation. Signs in the elevator tell you you are safe if the elevator stalls, you will not fall or suffocate. With some elevators you can hear the fan in the celing, with some there is not fan, but the shaft itself has a fan and fresh air source. There are openings behind wall and roof panels for air exchange. If people could see holes, they would lose keys, cell phones, false teeth, you name it, down the shaft, some by accident, some by kids playing, some by pnks grabbing and tossing.

bus. i used to take the bus and i hated it. smelly people would get on and i would gag on the smell. then foreign people would get on a talk loudly in some language, then as more people got on it would get really cramped, hot, stinky, smelly, and loud. and then i gotta move and make space for handicapped people and if wasn't careful people would steal my stuff. on top of that, i would need to walk 3/4 mile to wait for a bus (once every 1h10m) in whatever weather and then need to wait for he bus (in whatever weather), and then get on the bus and some handicapped person would get in and i have to give up my seat (even though i'm exhausted) because its the law. and i can't eat, drink, play certain music, and use the cell phone on the bus. the bus system is slow and inefficient, a 7 minute drive is a 4 hour bus ride (few bus route to take so i need to go the long way around), and they have item restrictions on what you may or may not take on a bus. So I hate the bus to death and would do anything to never ever board a bus ever again. a crowded elevator- ride is very short and generally well air conditioned. summer without ac? I'll buy a fan and then build my own ac system to I'd stay cool.

Elevators have ventilation fans. They're not airtight.

Why do you assume that there aren't any ventilation holes on elevators? That's rather presumptuous.

It's not like the elevators are completely sealed. There are gaps for some air flow. There's just no way to connect a/c or heating ducts to them, because they're moving so far up and down.

There are fans / ventilation in all elevators, plus an escape / access hatch. NO ONE has ever died of asphyxiation in an elevator.
