i was just wondering, my husband is the sponsor right? and then it ask military spouse(if applicable) and it ask for UIC and DOR and rank and stuff..but thats only if i were in the military, because there is a family section and it ask relationship, would me(his wife) go under family members, right??or military spouse?

Yes he is your sponcer. You are his dependent. If you are military yourself (enlisted/officer) then you of course need to feel out all information applying to that, other wise you just fill out the family/dependent section.

You are military the spouse, family members are for another deps. UIC is for his command.

Before long you will have all military jargon down...

Yep this is all true.

One word of wisdom is start to learn his Social Security number. That is what the miltiary goes by. You will always be asked for his SS# on almost every paper that you fill out onbase.

ok installation/organization transferred from is where you came from.....they want the base and the squadron for example i was at yokota ab japan before this so mine would says YAB 374th XXX and the transferred to is the base/squadron your at now. DSN is his duty phone number (the number you would call him at work if he wasnt deployed) AD service computation is the amount of time he has in. i hope you have a power of attorney though because the housing office wont accept that from you otherwise.

You can fill out the paper but its his info that goes at top and yours where it states spouse and dependents.

UIC is Unit Identification code.. his command as it were. that will determine eligibility for housing at the base.
