It's kind of like prejudice and discrimination. When you first look at someone, you have a feeling that they're smart because they look real nerdy, but instead they're not really smart. Or in the 1960's, people thought African Americans were... um..different.
When people "judge a book by it's cover", they mean you assume something about them by looks or the first time they meet you. Most people do this without even thinking about it. That's why people think first impressions are extremely important.
It can be taken literally, too. When looking for a book, people usually look for fascinating titles. They don't read the first couple pages first. That's judging a book by it's cover. It can sound not fascinating at all, but it might really be.

It means you're judging them by appearance and not getting to know what's on the inside. It's basically not taking the time to look beyond the surface, to simply take it at face value and look no further than that.
EX 1: He's butt ugly and no one will talk to him even though he's actually a funny, nice guy.
EX 2: She is the hottest thing on earth so everyone wants to spend time with her even though she's a selfish, mean jerk.
EX 3: She gets good grades and she's quiet so everyone thinks she's a nerd but she is actually the life of the party if you know her.
Judging a book by it's cover is merely looking at the surface and not going any deeper. That's one of the reasons we've had slavery, wars, and many other unfortunate events.

You see a person with tattoos all around his face and neck. You think he's a jerk or an idiot. That would be judging a book...... Because he might be the nicest, kindest person you've ever known. Maybe he was in a fire, and is covering a bunch of hideous scars. Or you see a girl with pink hair. You think she is weird. You are judging.......Maybe she's an actor on Broadway in a play and needs pink hair. Get it? Got it? Good!! :)

They are making assumptions about someone based on their initial appearance as opposed to their personality etc.
Like, if you see someone on the street in dirty clothes and unwashed hair you *may* think they're a bum. But there's a chance it could be a university student who just pulled an all-nighter while studying!
That's why people say, "You shouldn't judge a book by its cover!"

It means to decide if you like something on first viewing, without further study.
Ex.: That man drives a shabby car, and he's very poorly dressed. Who would have guessed he's the richest man in town? response "You can't judge a book by its cover."
She's very nice to old people and kids out in public. At home she slaps her mother and never feeds her children, depriving them of attention. response "You can't judge a book by its cover."

its simply making judgments on someones outside appearance, such as dress, skin color, hair color, eye color, ethnicity.
Instead of getting to know the person and what they are like as a person, you instantly do not like them for they look weird, strange different from you or other peoples you know.
I do not judge on outside appearances, as they are deceiving, but rather I judge on the content of a person character.

To automatically have an opinion on someone before getting to know them e.g seeing someone dressed in a certain type of clothing and assuming they are going to do something wrong,or seeing someone and saying "Oh I've seen her type before",you could be wright,but by doing this you are already building up an opinion on someone with knowing them individually.

Judging by skin color., employment, clothing style...etc....
It's judging by appearances and not personality. Stereotyping a person without actually knowing them as individuals.

The book is ugly on the outside, torn pages, messed up spine, but when you get to know the story inside, youll find out its another world different from what you first believe what it was.
Things are not always what they appear.
