You need to take off for one day to go to the doctor for a skin condition you might have and also follow up with a dentist appointment? I never take off of work. This would be my 1st day calling out in 9 months. How would you ask?

"I have two doctor appointments on (date and time), so I need to have that time off."

Just let your boss know that you will come in as soon as they are done, or that you need to leave a little early, whatever the case is. Try to schedule appointments early in the day or later in the afternoon to miss as little work as possible. If you give ample notice, I doubt there will be a problem, especially since you have a good track record.

You tell your boss or whoever at least a week in advance of the appointment that you need to take that day off. This gives him time to cover for you.

" I need to take next Friday off. I have a doctors appointment."

Don't just call in on that day, and say I won't be in . That makes people mad at you. Always give notice.

I don't ask, I inform.
"Boss, I'll be gone on the blabladate for medical appointments, I'll return at blabladate/time. Is there anything specific you want me to take care of before I leave?"
