Nathan, I love you = Kocham cie, Nathan. (Co-ham che Nathan)
I love Nathan = (Ja) kocham Nathana. ([Ya] co-ham Nathana)

When saying Kocham the stress is on the first syllable.

Kocham Natana or
Ja kocham Natana.

Natan, Natana, Natanowi. . .the names decline in Polish also, and there is no need for th, thus Nathan, becomes Natan.
Good luck.

Ja cie kocham, Nathan = I love you, Nathan. I suppose 'I love Nathan' would be 'Ja kocham Nathan.'
