I already have a certificate of approval.

Can I apply for this visa within the UK or do I need to go back to the British commission in my home country?

If you have a Form COA(MARRIAGE/Civil Partnership) Application for a Certificate of Approval to be shown to a Registrar for Marriage or Civil Partnership

This form should be used to apply for a Certificate of Approval.

The certificate is to notify a Registrar that the applicant intends to marry or register a civil partnership. This form is not an application for leave to remain in the UK.

So you now need to fill out
FLR(M) Application for an extension of stay in the UK as the spouse, civil partner or unmarried partner of a person present and settled in the UK as you weren't married before you came into the country.

You can apply for these forms here...

Apply in the embassy and you don not have to go to your home country. The embassy is the extension of your country.
