50 is not as old as the other answerers make it out to be. What are her interests? What activities is she into? Gift certificates to something that she's interested in is always a good choice. Or you can go with something like a gift card to something like Amazon.com which has a broad spectrum of things people can choose from since it is often hard to figure out what things a person already has, what sizes she may wear, etc. Or offer to take her out to a restaurant she's always wanted to go to but hasn't had the money or guts to go to yet.

How about making a scrapbook of family pics past and present with notations of special events and such or even a collage of family pics in a nice frame.

I like the spa day idea.

Gift certificate to Amazon.com or another store she likes, jewelry if you know her tastes, iPod, Sony Reader, kitchen gadgets if she likes to cook (one of my favorites is my ice cream maker), nice sweater, gym toys such as a fitness ball, charitable donation to an organization she supports.

Well try getting her a card and maybe some nice earrings (Jewelry). What does she like? A book, cooking utensils? Perfume? I'm not really sure, but I hope this sort of helped!!
