I have six tattoos, including a medium-sized tattoo on the top of my foot, a small one on my bikini line, and a monster of a piece that covers my entire lower/middle back (this one took about 22 hours over several sittings). The one on my shoulder blade was a blessing in comparison!
The point is, your flesh is being pricked over and over with needles. The final product is a wound until it heals, and most tattoos do "hurt" to some extent, but the pain level depends largely upon 1)Your personal pain tolerance 2)The types of needles being used and the person using them (some artists may go deeper than others), and 3)The area of the body you get the tattoo.
However, the beauty and meaning of your tattoo will far outweigh a little while of discomfort (how I would describe the sensation). Good luck!
P.S. The outline almost always is the most uncomfortable part-- a single needle is used, as opposed to a cluster of needles for the shading.

I have tattoos on both of my shoulder blades and neither one hurt very much. I don't have a very high pain tolerance. Also if it is a small design then you won't have to sit there for very long. I have both of my arms done and I think the longer it takes the more it hurts.

Don't do the shoulder blade!! It's not something you will want to see, THERE, FOREVER. Anything that you would ever see in a formal gown (ie. anywhere on your torso) is a mistake, for most people.
You might want to experiment by getting a henna tattoo (permant for a time) and wearing it around awhile, thereby getting a preview of what it's like. My husband just got a tattoo on a whim (didn't tell me before and I HAVE ONE too) and it's too large and not in a great place. TOO LATE NOW! Mine is very small and on the inside of my leg, above my ankle. Small enough to cover with a round bandaid (should I choose) and still very personal and to my liking. I have had it almost 20 years!

Depends on your pain tollerance, I guess. It's on your back, though, so it won't hurt as much as somewhere with more pain receptors, like the back of your arm. The back doesn't have many pain receptors.
