They can ASK, sure! Good luck trying to collect it, though.
I'd ask a lawyer in your state. Unless they paid through deliberate fraud on your part, I'd guess that you do NOT have to pay them back. But it's just a guess.

It's depressing, but the health care system in America just isn't a service, it's more like a privelege. I suppose the only reason to acquire it is in case someday you get a very serious and costly illness and that your insurance will (probably) cover. But little things like check-ups, annual screenings, lab work done after a check-up, even though they cost $300-$600 more than the deductible, insurance companies leave us in the lurch. We end up paying for most things ourselves, even if they said initially that they would cover something.

This depends on why they are now saying the claim wasn't covered. Is it because of a mix-up on their part? The disposition in that case should be included in the plan documents, so dig them out and do a good reading. You may not be responsible for reimbursing them.

If they are correct in that the claim was not covered under their policy, they can ask that you repay them.
This situation goes both ways. I venture that you would ask THEM to pay if you discovered that they refused to pay something in error.
Health insurance is a contract. You get what you deserve under the contract, and you give back what you got in error under the contract.
