No, I do not believe we've "evolved" from lower life forms, but rather that there is a God who created all of us as we are and all creatures as they are. I've several reasons for believing this. Primarily The creation account recorded in the book of Genesis tells me so, though I have further reasons to back this one up. In all my study into the subject of creation vs evolution I've never found any convincing evidence to support evolution, but plenty that lends it's self easily to creation. Animal skeletons used to "prove" evolution have time and again been shown to be two coexisting creatures, rather than one changing into another. Early homosapien skulls that have been reconstructed from the fragments found out in the field have so many holes in them that the pieces could easily be rearranged to construct a normal sized human skull. Carbon and other types of radio isotope dating methods have been shown to be flawed when they're used to date things who's age is known and are way off. Rock layers once said to take a year each to form have been shown to be formed much faster (a 300 foot section of them was created in a mere half-an hour). All These things cast a poor light on the theory of evolution as it is today. Natural selection however (being the adaptation of a creature to it's environment without the creatures becoming a new species entirely) is absolutely a factual, testable, and observable concept in modern science. Evidence such as the grand canyon and the numerous fossils found all over the earth line up well with the story of a global flood as recorded in Genesis as does the presence of a story of a global flood existing in some form in every culture known to man.
It's for these reasons primarily that I choose to believe in creation over evolution, and in the existence of a God who's created all we know. I hope this has answered your question adequately.

No, evolution is a hypothesis that has not been proved.
We have still yet to see any evidence of one species becoming another. Variations in the same species doesn't equate to evolution. For all we know at this stage is that those variations are preprogrammed in the DNA as possible variations. Mixing of DNA may make a new type of dog, but it is still a dog. So, even if a complex single cell organism managed to spontaneously form with perfect parts one time or even a thousand times, it wouldn't account for the wonderful variety of life here on Earth.

These are not exclusive, you can believe in God and also that we evolved from our decisions and choices over time.
There is a lot of evidence for this. Also many people with weak faith use questionable beliefs to support them. God creating the world 6K years go can be a crutch for someone who cannot love God without leaning on a permanent miracle. Creation should be enough, no matter how it is unfolding.

It has never been proven that an organism of entirely one type can spawn a creature of another race.
Those are only assumptions made after Darwin's observations that life adapts and changes over time. For instance: if a bird has a short, stubby beak, and the only food in its locale are bugs that live in little holes, over time, that species in that geographical location will grow a long thin beak to prevent extinction.
Notions like a raptor transforming into a chicken are absurd.

I believe there is a God, but not in the way we define it.
I don't believe in lower life forms. How do we know that we are more advanced than ants? Do we know the universe revolves around us, or us around it?
In short, I think there is a creator and we evolved. Interesting question.
