There are a number of ways animals can get worms:
1) eating an infected carcass (like a mouse or rat) that's infested with worms
2) Eating poo from an animal with worms
3) Tapeworms are transmitted by fleas
4) They can be born infected from their mother
As your dog is only 3 weeks old, it's most likely he got them from his mother - why on earth have you got a dog this young anyway? They're supposed to be with their mothers until they are at least 8 weeks, this dog will still need puppy milk formula!!
You need to worm puppies at 2, 4, 8 and 12 weeks and then monthly until 6 months old, regardless of whether worms are present or not.
Go to a vets and pick up some proper worming treatment. While you're there, ask about proper vaccination and fleaing protocols, when to neuter and how to care for such a young pup. Don't use pet store products.

Signs of worms include runny or bloody stools, a belly that swells up a lot right after the dog eats, and worms (alive or dead) in the dog's stool. (Tape worms look like little white flecks of moving rice; round worms usually come out whole.)
Dogs can get worms in different ways: round worms and whipworms can be passed from mother to puppy through the placenta at birth; dogs can get worms from stepping in or eating other dogs' stools (that have worms in the feces); heartworms can be carried by mosquitoes and fleas; tape worms are carried by fleas...
Your dog is old enough that it should have been wormed at least twice already. If the infestation is severe, over-the-counter wormers won't be strong enough to kill all of the worms.
Take a sample of the dog's poop to the vet (in a Ziploc bag) and have the vet check it. He can tell if the dog has worms and what kind of worms the dog may have... and can prescribe a wormer that will specifically target the kind of worms your dog may have.
Worms can make puppies very sick, so you need to treat your puppy as quickly as you can.

Symptoms: Seeing worms in the stool, diarrhea, weight loss, unhealthy hair lethargic, bloated tummy, vomitting...though in my experience it takes a SEVER infestation to show any symptoms.
They get them from other parasites, like fleas, from being around stool with the worms in them, from their mom or from other dogs .
Also, where did you buy a 3 week old puppy?! 3 weeks old is FAR too young. That baby should be with it's mother. At 3 weeks old the baby is still nursing. That is very very irresponsible for someone to sell a dog that young. That baby needs special care if it cannot be with its mother, please see a vet to see what you need to do to take care of it.

3 WEEKS!
are you kidding, where did you get a three week old dog from? it needs to be with its mother at that age, you dont get a dog for like, 12 weeks.
you must be mistaken i would hope.
they get it from birth, or they get it from fleas, or they get it from outside, ie stepping in other doo doo or near other doo doo.
if he has worms and hes a puppy, his belly is always fat and hard, then, look at his poop everytime, little white things in there wiggling around, thats worms, then hes not getting nutrients cause the worms are and he will eat a lot and the worms will get bigger and bigger and he will get very sick.
please tell my your dog is not 3 weeks old.

3 weeks old?? This dog is way too little to be away from his mother. Take him back so he can gain immunity to diseases and learn to be a normal dog.

Tape worms (in the poop) are flea larvae.
They have to have eaten fleas.
