I'm not sure how other trucking companies do it, but in my brothers company (which I am one of the mechanics in) we have one tire on the trailer (with rim) and one tire on the truck (without rim)
All mechanics on duty carry a 20 ton air jack, at least a 3/4 inch impact wrench, a air compressor, and tire tools for mounting and demounting a tire (a tire hammer, 2 spoons)
Something happens while we are on duty and they are in the general area, we'll get the call (this scenario is a time sensative load) we go out there jack up the trailer or tractor, change the tire, and they are on their way. If it happens while we are on duty in general area, (with a non time sensative load) we'll bring along the extra tire to remount on the rim that's coming off or put back on tractor.
How 'bout some real life senarios? I have personally seen trailers come back with two tires completely MISSING, tires rims and all... The studs were sheared off due to the force of the impact. The driver never felt anything, of course.
The strenght of the springs the tractor or trailer that the tire blows out on should support the stress, of one maybe two tires. So it should be fine, until the driver can get to somewhere he can handle it better.
Generally, if the driver is doing his job, he would make sure that all the tires have at least 100 psi of air in each. If the tire is under inflated, it will over heat and cause premature tire failure. And that ain't good.

Company drivers do not carry spares. A service comes out (with a spare) and changes them.
Owner operators sometimes carry 1-2 spares.

they call a place that comes out and changes it, you need a 20 ton jack and a 1 inch impact wrench to get those lugs off, sometimes a truck will carry one under the trailer because trailers usually ( esp with independant truckers ) are owned by another company,

1, they only carry the tire without the rim. The tires are not changed by the driver. They call for a tire service to come and change the tire when needed.

A truck is supposed to have one spare tire with them however a road service can bring tires with them if necessary.

if a standard car has one spare, i would think it would be most logical for the 18 wheeler to have 4, but probably not plausable.
What if they have more than one blow-out on the road, I'd want at least more than one.
