The form you require is a V778/1 and the cost of retention for 1 year is £105. It is £80 for every year thereafter and the cost for the year include the fee to assign it to a vehicle and you will get the information on how to do that when you receive your retention certificate. You should not require a visit to a DVLA office, unless you want to pick up the form personally.

The first link below is an information page on transferring/retaining

The second link allows you to apply online and the forms will be posted out to you. Just click the Retention Pack box ;-)

you need to go to your local DVLA office (a list is available on the website at this link http://www.dvla.gov.uk/contactus/localoffices/findnear/england.aspx)

You hand the form over with your log book, mot etc, - they keep it all and then post out to you the paperwork. When you receive the paperwork you have 24 hours to put the old plates on. Eventually you will get a new log book in the post.

BTW our previous friend is obviously not British!

Not sure what you mean by "retention". Are you referring to ordering a Vanity plate? If so, you will need to go to the DMV and fill out papers and probably pay for the plate. If you already have the plate and let the registration expire, you may not be able to keep that particular plate if someone else orders and pays for it.

just to add its NOT £80 a year to keep it on retention, its £20 or £25 (I have two or three, but I still don't remember the exact price).
Hope this helps.
